This is an opportunity to work with stroke survivors and their families to support them following a stroke
The service aims to identify and address the needs of stroke survivors and carer across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes. A Stroke Recovery Support Coordinator will:
- Support new stroke survivors and their carer in the community, from the point of discharge from hospital
- Provide personalised information, advice and support
- Support clients to make informed lifestyle changes which will help them to prevent further strokes
- Have excellent IT skills and demonstrate a flexible approach to your role.
The post holder will have experience/background in a caring profession ideally with experience of supporting people with disabilities.
The role is home-based with extensive travel across the local area and occasionally further afield.
Further information
30 hours per week, covering Reading and Wokingham. The use of a car with business use cover and ability to drive is essential to be able to fulfil the requirement of the role. Our services are contracted, we currently have funding for the contract until 31 March 2023.
