• Improving the health and wellbeing of patients through increased access to community based support and services.
• Increasing confidence and knowledge of patients to self-manage aspects of their lives that impact on their health and wellbeing.
• Increasing understanding of the benefit of engaging people in community prevention and peer support activities.

We are looking for someone with experience of managing frontline teams and the ability to provide the support and tools they need.

You will work together with RVA colleagues and voluntary sector partners to further develop a holistic and diverse offer across the town.
