We are seeking a Health Family Adviser to support people with a learning disability to maintain and improve their health and wellbeing.
People with learning disabilities are significantly more likely to have poor health than the rest of the population. A factor for this is that they face barriers and inequalities in health and social care which often lead to inadequate care and treatment.
The job holder will offer an information, advice and advocacy service along with practical support to people with a learning disability and their family carers in Reading. A key task will be to ensure people with learning disabilities over the age of 14 receive an Annual Health Check.
The post holder will also take referrals from colleagues in the Family Adviser team where there are health issues. This may include accompanying individuals to health appointments, ensuring appropriate follow up services are in place and working with other professionals to ensure, where needed, an appropriate decision is made if an individual needs a Mental Capacity Act assessment.
The job holder needs to be calm, non-judgemental, persistent, and a good communicator. The ideal candidate will have a background in health and/or experience working with people with learning disability.
