3 part-time positions – 19.5, 21 and 28 hours/week

Salary £22,781 p.a. based on 35 hours/week
Utilising your experience of delivering person-centred support, you will assist people who contact adult social care to identify their personal assets, interests and goals so that they can increase and maintain their wellbeing, boost independence, become more resilient and find solutions and support in their local community. You will also recruit, induct and support a small cohort of volunteers who offer an invaluable contribution towards delivery of the service.
What do you need to be a successful Community Connector?
- Experience of case work, supporting vulnerable people
- Experience of person-centred support planning, helping individuals to identify goals and achieve positive outcomes
- Integrity, empathy, compassion and emotional intelligence
- Confidence in using IT software, including relationship management systems. e.g. Mosaic
- Access to your own vehicle and a full UK driving licence.
