opportunity of growing and developing our services to patients, as well as leading our progressive
team?

The role will include:

• Leading the team working across all sites for the Primary Care Network (PCN)
• Growing and developing our health and wellbeing offering internally and externally
• Responsibility of having an overview of the team’s workload, ensuring caseloads are appropriately allocated
• Analysing, measuring, and reporting on the team’s work and impact with a view to developing and improving the services further
• Ensuring good channels of communication between team members and across the practice are maintained

If you have the Social Prescribing qualification (DNA Insight Social Prescriber Plus, or similar) and potentially have experience of working as a Team Leader, preferably in a healthcare setting, we look forward to hearing from you.
