Home from Hospital Coordinator – closing date 17 October 2021

One of the most rewarding jobs you can have is helping to settle an older person back in to their own home and to help them re-engage with the community after a stay in hospital.

Age UK Berkshire is working with Wokingham Borough Council to provide a short-term home from hospital service for Wokingham Borough residents.

The service offers practical support such as help with shopping, cleaning, GP appointments and ordering medication. The service lasts for 2-3 weeks.

Our experienced staff work to ensure that they are settled in at home as soon as possible after being discharged from hospital, and also give support with information, advice and signposting to other services that will help them in the future.

This role is to assist and coordinate the Wokingham Home from Hospital Service – ensuring that older people or people with a physical or sensory disability have practical and emotional support for a 2-3 week period following discharge from hospital.

This is a permanent contract and can be full or part time.
