Browns mental health assertive outreach worker – closing date 16 November 2026

Make a real difference. Browns is seeking a resilient, proactive Assertive Outreach Worker to deliver practical, person-centred support across Reading and Berkshire. You will meet clients in their homes and community settings, helping people with severe mental illness, and additional complex needs to move beyond crisis and build greater stability and independence.

As part of the Assertive Outreach (AO) teams in Reading, you will work alongside NHS mental health services and partner agencies. You will manage a complex caseload, coordinate risk and safeguarding plans, support clients with housing and benefits, attend probation or court appointments, and maintain continuity of care when someone enters prison or hospital. You will also contribute to accurate case recording and service reporting.

You will bring health and social care experience, an NVQ Level 2 or an equivalent qualification, confident communication skills and professional boundaries. You must be able to work calmly and independently. Experience in community mental health, including supporting people affected by psychosis, dual diagnosis or forensic backgrounds, is highly valued.
