An exciting opportunity to be part of a new service supporting older and vulnerable people when they leave hospital
Are you a good listener? Do you want to help connect people to voluntary organisations which can support them to stay safe and well when they leave hospital? Do you want to make a real difference for older and vulnerable people?
Reading Voluntary Action (RVA) and Involve Community Services are piloting a Hospital Voluntary Sector Navigation service to provide patients, their families, and staff, with information about voluntary and community services and activities which can support people when they are discharged from hospital. This builds on existing Social Prescribing and Community Navigator services in Reading and Wokingham.
RVA and Involve are each recruiting one part-time navigator for 25 hours a week, to start as soon as possible
The Navigators will work closely with staff at the Royal Berkshire Hospital and also visit patients in the three community hospitals in Reading, Wokingham and West Berkshire.
The successful candidates will be part of a new and exciting service, which is welcomed by our colleagues in the hospitals.The Hospital Voluntary Sector Navigator service may evolve over time as relationships develop and opportunities for closer working emerge.
This pilot is one of four additional voluntary sector services at the Royal Berkshire Hospital (RBH) over the winter period, supporting patients and staff, and aiming to ensure that patients can be discharged as soon as they are medically fit, can settle back at home with the support they need, and have a reduced risk of re-admission.
