Specialist Social Prescriber – closing date 24 May 2018

Working as part of the Reading social prescribing team, this post will enable older or vulnerable people to achieve positive wellbeing outcomes by helping them to manage their long-term conditions, and tackle social isolation by reconnecting them to their local community. Visiting people in their own homes, you will enable them to achieve outcomes that will improve their lives.

Experience of older people and issues that affect them is later life is essential. Due to the nature of the role, a driving licence is essential, as is access to a car.

The post is for 28 hours per week. Days and times can be flexible and will be discussed at interview stage. The post is initially for a 24 month period, but is likely to be for a period of 48 months.
