Social Prescribing Coordinator – closing date 30 April 2018

The Social Prescriber Coordinator is a new part-time role within an established and busy team, and will provide vital support for three (currently) part-time Social Prescribers.  The Coordinator will be office-based, keeping our online and paper records and database updated, receiving telephone calls about the service and contacting clients to make, and follow-up on, appointments.

About Social Prescribing

We aim to help people live well and safely, and to maintain healthy lives, through non-medical support offered by Reading’s voluntary and community groups. This in turn supports reduction in their use of GP surgeries, calls to 111 and 999, visits to A&E and avoidable emergency hospital admissions or reliance on adult social care.
