The Royal Berkshire Hospital NHS Trust is commissioning a service to support patients who are self-funding, to choose nursing/residential home accommodation or domiciliary care.

“In order to speed up patient discharge and increase bed capacity, there is a requirement for a selection service providing high quality assistance to self-funded patients seeking a nursing/residential home and/or domiciliary care package.

The Trust is seeking to appoint a partner to work within its Integrated Discharge Service to liaise with colleagues in Berkshire Health Care FT and Adult Social Services, who will provide advice to patients/relatives without charge to them, assisting with the selection nursing/residential home accommodation or domiciliary care.”

The specification details include:

  • Contract term: 3 years
  • Place up to 35 patients with mixed complexity per month, from RBH and as required, from community beds within West Berkshire.
  • Provision of support in managing the re-charge process between the Trust and the CCG.
  • Arrange and facilitate domiciliary care packages as an alternative to Care Home packages if directed by health/social care staff. The hospital and adult social care staff will always look to discharge to where the patient was admitted from and the team will support this aspiration.
