Brighter Futures for Children and Reading Borough Council have produced a guide designed to help parents and carers of young people with a wide range of abilities, including special education needs and/or disabilities (SEND), to understand the move from children’s services to adult services. Published in January 2020, the guide has been produced with the help of Reading Families’ Forum, Reading Mencap, Berkshire Healthcare Foundation Trust and the SEND Transitions Project Group.

The guide provides useful information to support planning for a young person’s future and covers:
